PRINT A CERTIFICATE OF PARENTAGE FORM

Instructions for Completing the on-line Certificate of Parentage

You can complete a Certificate of Parentage at the hospital at the time of the birth of the child, at the local registrar in the town that the child was born, at your county child support agency or you can complete the form and mail it in.

If you choose to mail it in, you must do the following:

The Certificate of Parentage must be printed as a two-sided document.

You must read all the information on the back-side of the Certificate of Parentage before signing it.

Once you have read it and decided you want to continue, please fill in all fields on the form; either type or print in black or blue ink.

Remember this is a legal document so it must be legible.

Both mother and father must sign the form and both signatures must be notarized. (Please Note: If the mother is married to a man other then the father of the child or was married at the time the child was conceived, an Affadavit of Denial of Paternity must also be completed.
